Unsworth Medical Centre – Changes to the triage process
(Effective Monday 17th March 2025)
The GP’s and practice team at Unsworth Medical Centre continually review our service to strive for more efficient and effective ways of delivering high quality patient care.
From week commencing 17th March 2025 we are going to adjust our ‘on the day triage’ service to run between 8am – 2pm Monday to Friday inclusive.
Should you feel you have a medical problem, that requires urgent attention on the day, please contact our team.
This can be done online via the NHS App, via the link on our website, via telephone or in person if preferred.
The reception team will ‘care navigate’ your enquiry – which means they will ask you a series of questions, which have been approved by the GP’s, to determine the most appropriate care for you.
If clinically appropriate you will be added to the telephone triage list and contacted by a GP on the same day. You may be asked to come in for a face to face appointment with a clinician on the same day. Please ensure you are available to attend the practice on the day you are requesting a triage appointment.
After 2pm the triage will close for the day and the reception team will continue to signpost you to the most appropriate clinician or add you to the triage list for the following day.
If your enquiry is routine the reception team will support you to the best solution whether this is scheduling a routine call with the doctor or an appointment to see another member of the practice team ie physio, nurse, pharmacist or offering you an appointment at local services such as community pharmacies, opticians or an extended access appointment which could be located across Bury at either Prestwich Health Centre, Radcliffe Primary Care Centre or Moorgate Primary Care Centre.
